How to Register
To register as a new patient, you must check that you live within our catchment area:

We are happy to accept new patients who are ordinarily resident in the UK and living within our practice boundary.

Register online
Please complete this online form to register with our Practice. Please check whether you are within our catchment area.
If you are registering in person, please be present with the registration forms and documents below:
- Proof of address dated within the last 3 months, such as a utility bill, council tax bill.
- Photo ID, such as: Driver’s License or Passport.
If you are on any regular medication, we ask that once you are registered, to get booked in with one of our Clinical Pharmacists.
You will appreciate that it may take a few weeks for us to receive your records from the previous doctor, and it is important that we know of any significant history or problem that you may be suffering from.
Please note that if you were previously registered with a Doctor outside the UK, the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

Carers
If you are the carer for someone who is elderly, frail, has a chronic illness, or has learning difficulties, please let us know so we can offer support.
Sutton Carers Centre provides a valuable source of help and information.
Change of Details
It is obviously important that we have the ability to contact any of our patients quickly in case of need.
Please help us keep our records up-to-date by letting us know if you change your name, address, or telephone number. If you move outside our Practice boundary, you will need to register with a local GP.
Organ Donation
From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information, please visit the Organ Donation NHS UK where you can also register your decision.
