Registration Policy

In order for us to accurately trace or allocate a National Health Number for a patient we require the following information.  This information will enable us to do an accurate and thorough search of the National Database and will prevent NHS numbers being duplicated unnecessarily. This also ensures we can trace your medical records.

Please ensure that the information detailed below is entered on the downloaded registration form correctly.

  • Surname
  • Forename/s (in full including middle names, no nicknames)
  • Title
  • Previous names (if applicable, most recent first)
  • Maiden name (for all married women)
  • Full address (including postcode)
  • Date of Birth
  • NHS Number (if available)
  • Sex Must be M or F
  • Telephone Number and Mobile Number
  • Place of Birth (town and county for patients born within the UK, country for patients from overseas)
  • We also ask for the date of entry into the UK for all patients from overseas (regardless of whether a previous address in the UK has been given.
  • Previous address (including postcode)
  • Previous GP’s full details (name, address and postcode not just the clinic’s name)

WE CAN ONLY REGISTER PEOPLE IF THEY LIVE WITHIN OUR CATCHMENT AREA

Although NHS guidelines state that you do not need to show ID, proof of address or immigration status, we ask patients to provide ID to ensure we have the correct patient details i.e that your name is spelt correctly on our records. We ask for proof of address to check that you live within our catchment area.

ID Required Original Only

  • Photo ID: Passport, Full Driving Licence, residence permit, ID card

Proof of address (dated within the last 3 months)

  • Council tax statement
  • Tenancy agreement
  • Bank statement
  • Utility bill

If you are on regular medication we must advise you that until your patient records have been received at this surgery you will only be prescribed a small supply of medication ie 1-2 weeks only.

Overseas Patients

Ensure that the country of birth and date of entry into the UK is on the form.  Please confirm if you have been to the UK before and supply us with the previous UK addresses along with original dates of entry and departure. 

You may be asked to show your visa, ID card or residence permit if you were not born in the UK.
